Sildenafil is used to treat male sexual function problems (impotence or erectile dysfunction-ED). In combination with sexual stimulation, sildenafil works by increasing blood flow to the penis to help a man get and keep an erection.


PRECAUTIONS

Before prescribing sildenafil, it is important to note the following: In patients on antihypertensive medication, sildenafil prescribed concurrently causes mean fall of BP 8mm Hg (systolic) and 7 mm Hg (diastolic). Its safety is unknown in patients with bleeding disorders patients with active peptic ulceration. The combination of heparin and sildenafil has shown, an additive effect on bleeding times in the anaesthetized rabbit but this interaction has not been studied in Humans. A thorough medical checkup is recommended prior to the use of Cupid, for some patients including those who have chest pain or other signs of blood flow to the heart, congestive heart failure and low blood pressure, are taking drugs to control blood pressure, have kidney or liver disease, and those on medication that can affect the actions of sildenafil.

DRUG INTERACTIONS

Sildenafil citrate is metabolized primarily by the cytochrome P450 (CYP) isoforms; therefore, inhibitors of these isozymes may reduce Sildenafil citrate clearance. Cimetidine causes a 56% increase in plasma sildenafil citrate concentrations, when the two are co-administered. Stronger CYP 3A4 inhibitors such as ketoconazole or itraconazole would be expected to have still greater effects, and population data from patients in clinical trials did indicate a reduction in sildenafil clearance when co-administered with the above-mentioned C.P.A. inhibitors. When co-administered with oral doses of amlodipine sive patients, the mean additional reduction of supine blood pressure was 8 mm Hg systolic and 7 mmHg diastolic. 5 mg or 10 to hypertensive patients, the mean additional reduction of supine blood pressure was 8 mm Hg systolic and 7 mm Hg diastolic. No significant interactions were shown with tolbutamide 250 mg or warfarin 40 mg, both of which are metabolized by CYP2C9. Sildenafil 50 mg did not potentiate the increase in bleeding time caused by aspirin 150 mg.
